At Hammond Park Secondary College (HPSC), we are committed to providing every student with the best support and educational experience. Recognising that each child is unique, we understand that some may benefit from external provider support. HPSC staff, families, and service providers work together to help students reach their potential, build meaningful relationships, and participate in their communities. With parent/guardian permission, we work closely with external service providers to ensure positive student outcomes.

This email is monitored regularly at scheduled times throughout the week, and actions related to service providers are handled promptly during these times.

For Parents/Guardians: Complete and return the 'Parent/Guardian - Request for Service Form'.

For Service Providers: Complete and return the 'Service Provider - Request for Service Form'.

Service Providers must also supply the following documentation with their Service Provider Request Form:

• A current Working with Children Check.
• A current NDIS Worker Screening Criminal Record Check or DOE Nationally Coordinated Criminal History Check Nationally Coordinated Criminal History Check
• Evidence of Public Liability Insurance that meets the Department of Education's requirements:
  • for an amount of not less than $20,000,000 for any one occurrence.
  • with an unlimited aggregate in public liability.

Note: Evidence of Professional Liability is no longer required to be collected by the school.

Please be aware that HPSC will consider requests in accordance with its duty of care to students and staff, students' educational and well-being needs, and providers' use of school facilities and resources.
